Occasional white spots as per image. 1/2000 f4 Auto ISO. Not present on all images from same shoot. Sensor and lens clean.  Any ideas?

Does it do it with different lenses?

Have you checked the inside of the lens for fungus/dust using a strong light? 

Have you checked for dust/spots on the sensor by closing down to f/22 or so and shooting at a plain white paper or bright blue sky? 

Unlikely it's hot pixels, nonetheless, how long since you've performed pixel mapping? The camera usually does it by itself, but with some settings you need to do it manually. Maybe worth a try.  

Everything I've suggested would be more possible if it happened all the time. The fact that it doesn't suggests something is moving around. I think the first question is important. If it doesn't do it with any other lenses, then it's clearly the lens.

Sorry about that. I believe pixel refresh is done occasionally on switch off when the shutter fires and I’ve cleaned the sensor with Visible Dust and cleaned front lens element-I don’t use filters. More tests to come but it’s the inconsistency that’s puzzling. Thanks for responses so far - appreciated

Correct, but as previously stated, if you use certain setting combinations it negates the auto-mapping. They tell you in the manual to do it manually every so often, maybe even once a week if your setup has those settings. Can't recall exactly what they are, it's worth checking out though.

Weird, I can't find it in the R IV. This page is from the R V manual. That doesn't mean it's not there, Sony tends to change their terms from iteration to iteration. See if it shows up when you do this:
