A77M2 ALWAYS out of Focus

Good day all, I'm new to this forum, this is my first inquiry as relating to the topic above, and I need your help.

I use the A77M2. I've noticed in recent past that my pictures (I shoot mainly Portraits) are NEVER in focus when shooting wide open mostly at 2.8 either with my Minolta 100F2.8 Macro or SAL-70200F2.8 G. Today because of the frustration I had to reset my camera to factory settings.

I mainly use Back Button Auto-focus.  some of my basic settings are: Shooting mode- Manual, AF-S/C, Focus area ranges from Wide, Flexible/ Expand Flexible/ 

lock-on AF-EF Spot, and metering mode is mainly spot. I also use the face detection. What could be the issue?
